Inter Milan - Ac Milan

some other good ones are:

Chelsea - Arsenal
Manchester United - Manchester City
Liverpool - Everton
Lyon - Marsielle
Barcalona - Real Madrid
Ajax - PSV
Chelsea - Barcelona (European Derby)

To me, these are by far the most passionate of the derbies.

A number of the suggestions on here aren't derbies. They are rivalries based on competition between the teams involved for trophies.

A derby is where two teams from the same area play each other. The important derbies are the games that the fans traditionally look forward to regardless of their league position or even if they are in the same league or not. For example,

West Ham v Millwall
Portsmouth v Southampton
Port Vale v Stoke

Would Liverpool v Chelsea be considered a derby if Liverpool were in the Championship? I think not, therefore, it's not a derby.

I agree with West Ham Millwall as one of the best. Ever seen Green Street Hooligans? It's largely about that derby.

But I'm biased towards Arsenal and Spurs.

Man United Vs Liverpool
Always intense and usually involve scrapping for the title. (Like todays 2-0 utd!!!!!!!)
Barca Vs Real isnt a derby but its still a great match as there are so many world class players on show(again played today)

Real Madrid vs. Barcelona is in my opinion one of the best derbies. I understand that for it to be a derby the two teams have to be close to each other. I would consider this a derby though because the rivalry between the two teams goes past football and the trophy to history. Franco suppressed the Catalans (Barca) and the Catalans were not allowed to speak there native language, catalan (duh). Now since Franco died the Catalans can speak there language freely again, but they still have not forgiven Madrid, since that is the Capital and where everyone who believed in Franco lived. Therefore I'm sure most Barca and Madrid supporters wouldnt care what standings their team is at, they just want to whoop the other team's *ss to get back at them.

its catholics vs protestans, republicans vs loyalists, british vs irish all in 90 minutes of football. the best derby, next one is ac milan-inter milan

Actually it could be considered a North-West derby. Manchester and Liverpool aren't that far apart you know. It's not like Chelsea vs Liverpool or Real Madrid vs Barca which are great games but nowhere near each other.
